What is acute care at home?
It’s in-home medical care for serious, time-sensitive conditions that don’t require the ER. It bridges the gap between office visits, urgent care, and hospital stays.
Is in-home acute care the same as home health?
No — home health provides follow-up or long-term support. Acute care at home is more advanced and typically includes real-time diagnostics, physician-guided treatment, and more immediate interventions.
What conditions qualify for in-home acute care?
We help manage COPD, heart failure, diabetes complications, chronic wounds, infections, and other conditions that may otherwise lead to ER visits or readmissions.
Will my insurance cover acute care at home?
Most acute care services are covered under Medicare, Medicare Advantage plans and many commercial plans. We can help confirm eligibility and coverage before starting care.
How quickly can you respond if my condition gets worse?
Our triage team can typically assess new concerns within 24–48 hours, sometimes sooner, depending on urgency and region.
How is this different from urgent care or a telehealth visit?
We come to you in person with advanced diagnostics, wound care tools, and lab capabilities and build a full care plan based on your condition and history. Follow up includes our telehealth service.
Will you work with my primary doctor or home health nurse?
Yes. We always coordinate with your existing providers and share updates, notes, and care plans to keep everyone aligned.
What if I have multiple conditions or medications?
Our multidisciplinary team specializes in managing complex cases. We assess the whole patient, not just the symptom.

Michael G. Gonzalez, MD, FACEP
Medical Director, Acute Care Program – Houston
Dr. Michael Gonzalez is a nationally recognized emergency medicine physician with over two decades of clinical leadership experience spanning civilian, military, and telehealth environments.
At Restore First Health, Dr. Gonzalez leads our Acute Care Program in Houston, TX, driving our commitment to exceptional patient outcomes, collaborative care, and advanced mobile therapies. Through an elevation of partnership and discharge alignment with health systems, Dr. Gonzalez and the mobile clinical team work to support patients in the comfort of their own homes when advanced therapies are required to reduce admissions and readmissions for patients with chronic diseases.
He earned his M.D. from UT Houston Medical School and completed his residency in Emergency Medicine with the U.S. Air Force. As Chief of Emergency Services at Landstuhl Regional Medical Center, he directed critical care air transport teams across Europe and the Middle East.
Dr. Gonzalez is best known for spearheading Project ETHAN, an innovative telehealth model reducing unnecessary ER visits while improving patient care. His work has earned him accolades, including the JEMS/EMS Innovators Award and multiple military commendations.
Dr. Gonzalez lives in Houston, Texas, along with his spouse, Dr. Anne V. Gonzalez, who is a practicing Ob/Gyn in the area, and two teenage boys. As a family, they enjoy travel, exploring new restaurants, pickleball, and racing all over the State to keep up with their boys’ swimming, soccer and climbing endeavors.

Jonathan Tibaldo, MSN, APRN, FNP-C, ENP-C
Lead Advanced Practice Provider, Acute Care Program – Houston
Jonathan Tibaldo began his career in healthcare in 2000 as an EMT, an experience that sparked his passion for emergency medicine and ultimately led him to pursue nursing. He earned his Bachelor of Science in Nursing from The University of Texas Medical Branch in 2005 and spent the majority of his nursing career in emergency care settings.
In 2014, Jonathan returned to UTMB to complete his Master of Science in Nursing and became a board-certified Family Nurse Practitioner. He later earned a second board certification as an Emergency Nurse Practitioner, reflecting his continued commitment to clinical excellence in acute care.
Jonathan has over two decades of healthcare experience and has served as a Nurse Practitioner in both emergency departments and mobile urgent care settings. For the last five years, he worked with a mobile urgent care, including three years in a leadership role as Lead Advanced Practice Provider, where he helped drive operational efficiency and clinical quality across his team.
A lifelong Texan, Jonathan grew up just southeast of Houston and enjoys rural life with his husband, Jaime, and their three children. Together, they maintain a small farm with a thriving garden, a flock of approximately 60 chickens, and a herd of 12 cattle.
