Mobile Wound Care

Our Program
Restore First Health offers a mobile advanced wound and care coordination program for the treatment of complex and chronic wounds.
Utilizing the most innovative technology available at the bedside, our team of physicians, nurse practitioners, nurses and medical assistants create a comprehensive plan and ensure a holistic approach to the patient – and their wound.
Conditions We Treat
We care for complex and chronic wounds:
We collaborate with community partners to offer solution driven approaches to the most complex cases.
Cornerstone to our Program: Care Coordination
From the first phone call or referral to our initial at home evaluation and therapy, we inform, communicate and ensure all of our patients and their care providers are on the same page at all times.
Our dedicated wound care coordinator:
Schedules consultations and appointments
Verifies and pre-authorizes insurance
Communicates with the patient’s current care physicians or providers
Provides medical updates to authorized providers for continuity of care
Regular check-ins to ensure quality of care and positive outcomes
The RFH Difference

We come to the patient for evaluation and treatment.

We evaluate appropriate supplies and DME

We verify and authorize insurance on behalf of our patients

Evaluate and establish coordinating medical experts for chronic disease management

Through the entire process, we coordinate with the patient’s PCP or Home Care Provider to work in concert with all of their overall healthcare needs.

We are committed to excellence in wound care and document progress each step of the way to share with the patient’s clinical team.

After working with us:
Our patients become educated on ongoing care
Feel that their caregivers have the support they need
Our patients feel they were given personalized care
The right care for our patients is what matters most – we’ll always find a way.
Partnerships, community relationships, and exceeding the needs of our patients through quality wound therapy is our commitment to better health.
