Our Strategic Account Manager, Reyzury Jackson Rogers, proudly hosted the Clayton County Senior Alliance Networking Meeting at our Riverdale, Georgia office, bringing together health care professionals, senior service providers, and community partners who share a common goal: improving access and outcomes for seniors across the Atlanta area.

The Clayton County Senior Alliance provides an important platform for health care and senior service professionals to connect, share resources, and discover better ways to serve older adults in our community. These gatherings spark meaningful partnerships, allowing providers to align on care strategies, enhance referrals, and ultimately reach more patients who need support.

Connecting Care to the Community 

As our team welcomed community partners into our Riverdale office, the discussion centered around how collaboration can close the gap between hospital discharge and home recovery, reduce readmissions, and improve quality of life for seniors living with chronic or complex conditions. 

Through our mobile health care programs, we bring care directly to patients, ensuring they receive the treatment, education, and follow-up they need without the barriers of travel or access.
