Restore First Health’s Continual Monitoring & Support Process

Chronic swelling and lymphedema require more than occasional visits. They require structure, coordination, and long-term clinical oversight.

Restore First Health delivers Advanced Mobile Lymphedema Care directly in the home — combining evaluation, compression therapy, monitoring, and provider communication into a clear, step-by-step pathway .

Here is how our Lymphedema Program works.

Evaluation & Treatment

Every episode begins with coordinated access and comprehensive evaluation.

We provide:

  • Insurance verification and rapid scheduling
  • Comprehensive in-home evaluation
  • Individualized treatment plan and compression garments ordered
  • Delivery and in-person education for patient and caregivers

Our clinicians assess volume changes, mobility, skin integrity, and risk for wound development. We coordinate with PCPs, home health agencies, and specialists to ensure the care plan is aligned.

This is where stability begins.

Continual Monitoring & Support

Lymphedema management is not a one-visit solution. It requires structured follow-up.

Restore First Health follows a timeline-based monitoring model:

1-Month Clinical Reassessment

At one month, we:

  • Evaluate early progress and adjust treatment (via telehealth when appropriate)
  • Return updated documentation to referring providers
  • Assess ongoing in-home compression usage
  • Take clinical measurements and coordinate with PCPs, home health, and specialists

Early detection of setbacks helps prevent complications, skin breakdown, and avoidable hospitalizations.

3-Month Outcomes Review

At three months, we:

  • Reassess volume reduction, wound progression, compliance, and stability
  • Modify care plans if needed
  • Perform compression and device reassessment
  • Reinforce patient education
  • Continue monitoring and therapy

This structured checkpoint protects progress and ensures the patient remains stable at home.

6-Month Comprehensive Evaluation

At six months, we:

  • Determine maintenance readiness
  • Assess and reorder compression as needed
  • Detect early signs of setbacks
  • Maintain ongoing provider communication
  • Reinforce self-management strategies

Our goal is long-term edema control — not short-term symptom management.

12-Month Program Review

At one year, we:

  • Assess long-term outcomes and confirm stability
  • Conduct an in-person comprehensive evaluation
  • Perform a Quality of Life assessment
  • Establish a long-term follow-up plan
  • Continue education and provider communication

This full-year review ensures sustained success and reduces the risk of regression.

Why This Process Matters

Lymphedema and chronic swelling are often under-managed. Without structured oversight:

  • Swelling worsens
  • Skin integrity declines
  • Wounds develop
  • Hospital visits increase

Restore First Health integrates:

  • Care Coordination
  • Communication
  • Collaboration

We act as an extension of your clinical team — not a replacement — ensuring continuity across home health, specialists, discharge planners, and primary care.
