Restore First Health (RFH), an advanced mobile wound and care coordination program, embarks on impacting the Jacksonville, Florida community through expanded services in the region. RFH is a national program specializing in advanced therapies for those people suffering from complex and chronic wounds. The mobile program collaborates with home health companies, health systems, skilled nursing, social workers, discharge planners, physicians, and families to bring advanced care into the homes of patients who have difficulty mobilizing and healing.
“From our early beginnings several years ago, we knew that patients in Florida would benefit from this critical at-home service,” said John Fish, CEO of Restore First Health. “Building on the partnership approach across our other regions we serve, we have seen much success in helping to identify and manage difficult cases that need a lot of hands-on care.”
Utilizing the latest technology and wound procedures, advanced practice nurses work side by side with home health companies to coordinate wound care and identify the holistic needs of patients, including vascular support, nutrition, and managing comorbid conditions. RFH focuses on developing relationships with other community providers including health systems, physician specialists and more, enabling synergistic care for patients who often struggle to mobilize to clinics or hospitals.
“In 2023, we published the results of our Community Health Assessment for Duval County and one of the findings from our study was the need to increase management for chronic diseases in our community”, Tina Robbins, Business Development Director, Jacksonville Chamber of Commerce. “Our community sprawls far and wide, creating problems for patients to access the care they need and properly manage their diseases. Restore First Health is bridging a gap in the healthcare for our residents and allowing us to continue to make positive changes for care management. This is exactly what we need for the people of Jacksonville.”
“When a provider or family member identifies the need of a patient with a wound in our community, it’s our goal to make the communication and coordination as seamless as possible and be the easy button for specialized wound care”, said Wendy Smith-Haupt, Vice President of Operations for the Southeast.
